§ 14-397. Application.

Application for the license required by this division shall be made under oath to the city by the owner or lessee of such machine or mechanical device, upon forms to be provided by the city. Such application shall correctly describe the premises upon which the machine will be used and include the name of the owner of the machine or lessee of the machine, shall include a statement that a sub-certificate of registration has been issued with respect to such machine by the state department of revenue pursuant to the provisions of the Illinois Retailers' Occupation Tax Act (Ill. Rev. Stat. ch. 120, ¶ 440 et seq.) and that the sub-certificate of registration is attached to such machine in a conspicuous place as required by law, and shall set forth the registration number assigned to such machine by the state department of revenue.
